Additional person MUST be in the same household and included in the plan on the same day. Full Mouth Debridement is not included in the no charge cleaning. Bleaching services, bite splits, and any orthodontic treatment (i.e. invisalign or six month smiles). Cannot be used in conjunction with any other dental plans or insurances.
