If you have dental insurance, bring your current insurance card. If you dont have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
X-Rays If a previous dentist has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
