Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

We accept Mastercard and Visa credit cards as well as cash and personal checks. We accept many insurance plans including all PPO plans. We are contracted in network with Cigna Radius, Assurant, United Concordia, DELTA Premiere, GEHA, BCBS of Florida, and Guardian. In addition, we accept financing options through CareCredit. My teeth were cleaned very good. Everyone was really nice and helpful during my dentist appointment. Dr. Chiafair was funny but he also educated me about the things that I needed to do to help keep my teeth healthy. I definitely recommend coming here.

First time going to a dentist in Jax since moving from NC. Normally going to the dentist is a morning filled with anxiety. Not the case this morning. Such a great experience with Dr. Chaifair and the staff he has. Walked me through every step, answered all of my questions with details. Went in for a cleaning and still catered to a last minute need (and want) on the fly. If you are looking for a great experience with a dentist this is your place!!!

Excellent service professional staff Dr. Chiafair is terrific dentist and is concerned about his patients I recommend His practice highly. He preformed implant surgery 8 years ago and I have had absolutely had no problems.
