Dr. Sivik is a native to Mentor, Ohio and graduated from Mentor High School in 2000. After high school, he attended Case Western Reserve University where he played varsity soccer and graduated with degrees in Chemistry, Sociology and Spanish in 2004. He then attended the Case Western School of Dental Medicine and graduated in 2008. During his 4th year of dental school he took over 150 continuing education hours learning to place and restore implants and is now a fellow of the International Congress of Oral Implantologists. Immediately following dental school he began working as an associate at Ashtabula Dental Associates and has not looked back since. He and Dr. Coliadis purchased the practice from Dr. Sockman and Dr. Seymour in July 2014 and now continue the service that Ashtabula Dental Associates has provided to northeast Ohio for over 50 years.
Dr. Sivik met his wife, Sarah, in High School and they married in 2012. Sarah is a stay at home mother with their three sons, Cole, Landon, and Beckham. Recreationally, he enjoys spending time with his family, traveling, soccer and any outdoor activity.
Professionally, he focuses on providing the best dental care possible. He is a key member of the team at Ashtabula Dental, where general dentists and specialists work hand in hand to provide a great patient experience. He has been treating patients who suffer from sleep apnea since 2016 making custom oral appliances to help maintain a patients airway. He also treats patients who suffer from chronic migraines, head or neck pain, muscle trigger points, or severe pain from clenching or grinding with trigger point injections with lidocaine or Botox/Xeomin injections.
He has recently taken over 100 continuing education hours in Dental Sleep Medicine helping those who suffer from Sleep Apnea with appliance therapy. He is also certified in Invisalign Orthodontics. Dr. Sivik is a member of the American Dental Association, Ohio Dental Association, Northeast Dental Society, American Academy of Dental Sleep Medicine, American Academy of Facial Esthetics and the International Congress of Oral Implantologists
Dr. Coliadis was born in Warren, Ohio. His family moved to mid-michigan early in his life and he graduated from Grand Blanc High School in 2000. After high school, he attended Eastern Michigan University on a prestigious Presidential Academic Scholarship, and graduated with a degree in Biology in 2004.He then attended The Ohio State University college of dentistry, following in the footsteps of his brother Nicholas Coliadis.Jason graduated in 2008 and enrolled in a year-long residency at Miami Valley Hospital in Dayton, OH. Following his residency, he began his career at Ashtabula Dental Associates in 2009. As Dr. Sockman and Dr. Seymour approached retirement, Dr. Coliadis and Dr. Sivik purchased the practice in 2014 and have been continuing the traditions set by their predecessors.
Dr. Coliadis met his wife Jennifer at Eastern Michigan, and they married in 2009. Jennifer is a graduate of the Wright State University College of Medicine, and is a pediatrician at PediatriCenter of Greater Cleveland. They are passionate supporters of the Upside of Downs, a nonprofit dedicated to enriching the lives of individuals and families affected by Down Syndrome. They have two children, Brandon and Caroline. At home, he enjoys playing and recording guitar, sculpting and woodworking, and spending time with his kids. He is a blue belt in Brazilian Jiu-Jitsu.
Professionally, his interests involve oral surgery and dental care for the special needs population. He is a member of the American Dental Association, the Ohio Dental Association, and the Northeast Ohio Dental Society.
Dr. 'Jay' Bader DDS is a native of Ashtabula,graduating from Ashtabula High School in 1973. He went on to achievea bachelor of Arts from Miami University, and ultimately graduated from The Ohio State University College of Dentistry in 1980. He has been an institution at Ashtabula Dental Associates for many years, providing general dentistry to patients of all ages.
His professional interests include cosmetic dentistry and esthetics. He is considered our resident 'Lab Master', providing expertise in denture and partial maintenance and repair. He is a member of the American Dental Association, the Ohio Dental Association, and the Northeast Ohio Dental Society.
It is with excitement and gratitude that I join the Ashtabula Dental Associates team! I was born and raised in the city of Ashtabula where my father has worked for 45 years as a train conductor for CSX and my mother has worked for After School Discovery since its conception. I was a 2010 graduate of Lakeside High School. After high school I went on to attend Marietta College in Marietta, OH where I studied biochemistry and played varsity womens soccer for 4 years, graduating in 2014. This brought me to Columbus where I attended The Ohio State University College of Dentistry, and graduated with my D.D.S in May 2018.
I am no stranger to Ashtabula Dental Associates as this has been my dental home since I got my first tooth Dr. Seymour as my family dentist, Dr. Forsberg as my orthodontist, as well as patient to many of the outstanding hygienists. Growing up, Ashtabula Dental Associates was always a friendly and comforting place to be and from these experiences, I declared that I wanted to be a dentist at the ripe age of 8 years old. Once I shared this revelation with Dr. Seymour, he went on to provide me with exceptional mentoring and allowed me to shadow him throughout high school and college. This community has truly shaped me into who I am and has given me a great support system, hard work ethic and appreciation for working in a small community. I am proud to re-join the community of Ashtabula and provide dental care to so many of the hard-working individuals and families that I have grown up around.
In my free time you can find me enjoying anything outdoors-- whether it be skiing or perch fishing on Lake Erie, playing soccer or playing with my golden retriever, Zelda. Professionally, although I am waiting to find my particular niche, I appreciate every aspect of general dentistry and family dentistry is always an area that I wanted to pursue. I truly enjoy treating all ages and I aim to provide the best dental care possible. I have most recently held the position of student team dentist for Ohio State Athletics, (providing preventative and emergency dental care and making mouthguards for their athletes) and I am certified in Invisalign orthodontics.
