Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Almond Orchard Dental is a very professional and educated Dental Office! I've been very diligent about my cleanings in the past, yet in one year with Almond Orchard I have learned more about preventative dental than ever before. My dental health has improved and I've learned how to maintain my progress. I wish I would have found them 20 years ago

My husband and I have been with almond orchard dental for many years. The staff is like family. They are all very professional and very kind. They always let us know up front what we need to get done . And, they always spend the time to help us feel comfortable with the process. I highly recommend them for all your dental needs.

Almond orchard dental care saw me on the same day that I had an emergency!! my lip was swollen up from a infected tooth and Dr. Alan did an amazing job with everything. He also took time out of his day to give me a quote for a implant and a snap on denture !! I'm impressed was how everything went and with the quote and I hope I get to go there to get my implant & snap on denture done .

Almond Orchard Dental, and Dr. Alan Tanner came into my life in a crisis. After just moving to the area, I had a back molar split all the way to the root, and needed an emergency extraction. My treatment and after care were exceptional, including personal calls to check-in post operatively. Since then, I've had Invisalign, an implant, and all my routine dental care through Dr. Tanner and his caring and attentive team. I highly recommend this office and Dr. Alan Tanner for all your dental needs!

Been visiting this office for a few years now... would not go somewhere else if i was paid to (and I'm broke!). I love the ladies that meet me when I come in. Always put a smile on my face and put me at ease. My DUDE, Alan, is the best. He always has the best tunes playing in the background, and if lucky enough he'll start singing. MOST DEFINITELY my kind of dental office. Ohhhh, and the doctors are pretty cool too. Kidding! They are awesome. Doc Senior is whom I regularly see. He makes me feel at ease while he checks to make sure my Old Skool silver fillings are still holding up. All in all, this is a hands down choice for me. Thank you guys for helping me keep my smile! Your team ROCKS

Staff and medical/dental professionals are all friendly and professional. I especially appreciate their understanding of my insurance situation after being laid off. There were several options given to me for payments and treatments. Throughout the treatments and procedures everything was explained to me step by step. When the pharpharmacy had issues filling the RX the doctor was available several times that evening to make sure I got my medication filled when I needed it. The office and doctor followed up with me for the first few days.

Dr. Rasi is the best! I've been going to her for almost 20 years. Doctor Tanner is also great, but I'm not his patient. Everyone I have dealt with in that office is great. I look forward to going there a few times a year. They feel like family. Everyone I have dealt with at that office has been wonderful, but I have to say that Lindsey has been amazing, she has always helped me understand treatments, cost, etc... I recommend them in every way. I travel 40 min at least, because I don't want to switch dentists!
