Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

After a long search for a dentist, Dr. Kuznick was recommended to me by a family member who has been a patient of his for over 30 years. I had many significant dental issues and have always been sent out to various specialists. Dr. Kuznick was able to treat all of my issues in his office. He has done extensive work on my teeth and gums. I basically had my whole mouth restored. Dr. Kuznick has treated me with kindness, patience, compassion and professionalism throughout the whole process. I am so pleased with the results and have received so many compliments on my beautiful teeth. I would also like to compliment the office staff and Dr. Kuznick's assistant Cynthia. I have been treated so well by all.

I remember the big smile on this dentist when I first walked in for a major dental work. Gave me full mouth exam to determine the entire cost. Halfway into the job, after everything was ripped apart, I was told he couldn't go on unless he did the lower right section of my mouth, too, which was not included in my budget. What was the full mouth examination for then? The job lasted 6 months and at the end of it, gathered all his staff staff to tell me how beautiful it looked, never mind what I thought. I complained about so much discomfort; the fit just didn't feel right. The smile that I saw when I first walked in was completely wiped out from this dentist's face; his tone of voice was angry and defensive. And after paying in full, which included the so called "unforeseen" job considering he did a full mouth examination before starting the job, the best he could say was, "if I wanted it fixed it's gonna cost me.!" Life isnt' fair, is it?

Dr. Kuznick and Cynthia are absolutely phenomenal. They are kind, compassionate people who truly love what they do and it shows. He takes time, care and pride in his work. For most of my life, I was self conscious about my teeth and Dr. Kuznick transformed my smile and gave me new found confidence and I now smile brightly and constantly. I would and have recommended him to all of my family members and loved ones because I know they will be in good hands. I now love going to the dentist and they even have techniques that make taking xrays easier (which used to be my least favorite part of going to the dentist). I can't say enough about this office, they are lovely.

Dr Kuznick and staff are exactly what u want and need in a dental office! When u enter the office . Any fear you have for going to the dentist. Is gone!! Everyone there are friendly and care about your well-being. There's never a feeling that they dont have your best interests at hard. From the front to the back. Amazing experience. Dr Kuznick is an amazing dentist. He will spend as much time as you need explaining and answer questions. I promise you!! You will be happy with his work!!

Dr. Alan Kuznick is one of the most thorough dentists I have worked for. He treats everyone like family and with kindness, in the most professional manner. From aesthetic dentistry to cosmetic dentistry, he does it all with modern dentistry. All equipment is up to date and Digital. Not only will you leave with an absolutely perfect smile you will love, I can almost guarantee you will want to come back just to maintain it all ! I have been a patient of Dr. Kuznick's for many years. He is a consummate professional who deeply cares about his patients well being. Each time you visit the amazing staff greets you warmly, you wait maybe a few minutes, then you have Dr. Kuznick's full attention. His gentle touch and attention to detail guarantees that you will have a positive experience. Though I had many challenges as a 60+ Invisalign patient, he worked with me to get through it.
