Drs. Bhola, Kolhatkar, & Cabanilla Jacobs are specialists in Periodontics and Implantology. We specialize in conservative and surgical treatment of gum disease including cosmetic procedures to aesthetically enhance your smile, as well as dental implants that can replace single or multiple missing teeth.

Although no one likes to go to the doctor or dentist, we strive to make your visits with us as comfortable and pleasant as possible. We strongly believe that our patients comfort is an important part of quality dental care. We work closely with your dentist, but a referral by a dentist is not required to visit our office.

A periodontist is a dentist who specializes in the prevention, diagnosis, and treatment of periodontal (gum) disease and in the placement and maintenance of dental implants. Periodontists are experts in the treatment of all forms of periodontal disease, including gingivitis, periodontitis, and gingival recession.

The American Dental Association recognizes periodontics as one of the nine specialties of dentistry. Periodontists receive extensive training, including three years of additional education beyond dental school.

Periodontal treatment is necessary when various conditions affect the health of your gums and the regions of your jawbone that hold your teeth in place. Retaining your teeth is directly dependent on proper periodontal care and maintenance. Healthy gums enhance the appearance of your teeth, like a frame around a beautiful painting. When your gums become unhealthy, they can either recede or become swollen and red. In later stages, the supporting bone is destroyed and your teeth will shift, loosen, or fall out. These changes not only affect your ability to chew and speak. They also spoil your smile.

Periodontal diseases are ongoing infections of the gums that gradually destroy the support of your natural teeth. Periodontal disease affects one or more of the periodontal tissues: alveolar bone, periodontal ligament, cementum, or gingiva.

While there are many diseases which affect the tooth-supporting structures, plaque-induced inflammatory lesions make up the majority of periodontal issues and are divided into two categories: gingivitis and periodontitis. While gingivitis, the less serious of the diseases, may never progress into periodontitis, it always precedes periodontitis.

Dental plaque is the primary cause of gingivitis in genetically-susceptible individuals. Plaque is a sticky colorless film, composed primarily of food particles and various types of bacteria, which adhere to your teeth at and below the gum line. Plaque constantly forms on your teeth, even minutes after cleaning. Bacteria found in plaque produce toxins or poisons that irritate the gums. Gums may become inflamed, red, swollen, and bleed easily. If this irritation is prolonged, the gums separate from the teeth causing pockets (spaces) to form. If daily brushing and flossing is neglected, plaque can also harden into a rough, porous substance known as calculus (or tartar). This can occur both above and below the gum line.

If gingivitis progresses into periodontitis, the supporting gum tissue and bone that holds teeth in place deteriorates. The progressive loss of this bone, the alveolar, can lead to loosening and subsequent loss of teeth. Periodontitis is affected by bacteria that adhere to the tooths surface, along with an overly aggressive immune response to these bacteria.

Periodontal disease is dangerous in that it is often painless and symptomless. 80% of Americans will be afflicted with periodontal disease by age 45, and 4 out of 5 patients with the disease are unaware they have it.

Dr Shilpa Kolhatkar DDS, MDS is amazing. She gave me two beautiful implants and all is now good. No more "holes in my mouth". Staff is efficient and caring.

Dr. Bhola was amazing! I had so much anxiety going in for a gum graft procedure. First consultation he explained everything he was planning on doing during the procedure. Still having little anxiety ..surgery day came. Once he numbed me I felt nothing. I didn't even feel the numbing needle either. The whole procedure was smooth and easy. The recovery was surprisingly easy as well. I felt no pain afterwards! I was so grateful to have did it and not put it off. He simply is the best. Hands down. Would definitely refer him to friends and family. Thank you!

I just had an appointment last Friday, September 2, 2022 at Advanced Periodontics and Implantology in Plymouth. I submitted a Google Review a few months ago and can only say again, I am so happy to be receiving treatment from the office of Dr. Kolhatkar. Again, she and her staff could not be more professional and caring. They each listen to you and take into consideration any concerns that you may have and wish to discuss. They strive to keep you comfortable and provide state-of-the-art treatment quickly, safely, and effectively. If you are one of their patients, you will ALWAYS experience the finest professional care. And again, I cannot say enough about Dr. Kolhatkar's professionalism and kindness to ALL of her patients. Most of my visits are with the Dental Hygienist, but Dr. Kolhatkar will always make sure to take a moment to say, "hello" to you before you leave her office. Lots of people are very fearful of going to any dental office, but after your first visit at Advanced Periodontics and Implantology in Plymouth or Livonia, you will never feel that way again. She and her staff are the best and genuinely want only the best for you too.

My experience with API is always a good one. The wait time to see the hygienist for my cleanings is very minimal and my teeth always feel so fresh and clean! The hygienist makes sure to explain the oral hygiene instructions and is never in a rush to get me out of the chair. It really makes going to the dentist a great time. I highly recommend them for all of your cleanings.

I recently had another dental implant done by Dr .Kolhatkar and couldn't be more pleased. The service was excellent and they accommodated all my needs. Dental work is never pleasant but they certainly made it more manageable! They are all so pleasant and totally professional. I highly recommend Dr. Kolhatkar and the staff at Advanced Periodontics!

Because they are so busy don't expect a lot of time with the doctor. They hustle you in and out. The assistants can be a little short with you at times as well. They get frustrated with you for asking too many questions. Also, be prepared to pay a deposit. I actually lost an appointment because I wasn't prepared to pay the 10% deposit on the spot (over the phone). Then, to reset the appointment, I had to pay a 20% deposit and miss a day of work. I was also sold a full mouth 3D scan. Which It turns out, I didn't really need. Which is what happens a lot of with these specialists. They always want to be paid something to see you. I felt like she used fear and intimidation to get me to go along with an expensive procedure. I was thinking of not doing the implant and just having a wisdom tooth pulled. She refused to do it unless I did the implant as well. The plan was to fix my number 2 tooth which didn't happen. I felt like she let it go so that I would eventually need another implant. Also, I felt like she wouldn't fix it because of the time it would take. So on a follow up visit the talk started immediately about coming in and getting more implants done! I felt like a cash register there. I had to go into debt to get this one done. And get this, she didn't prescribe any pain medication! Two years later when I was ready for my next implant I set an appointment only to have them call me to cancel it. So apparently they only want good reviews not real ones. Go to Plymouth Dentistry on Ann Arbor Rd. Better prices and better service. I've had 3 done there and getting number 4 in a few months. Also had a new crown put on, great job.

Amazing Staff! Dr. Bhola and his office are top notch. He is extremely knowledgable and dedicated to his work. He is ,imho, the BEST. My implants were performed painlessly and were very well done. All procedures were explained in detail in a very relaxed atmosphere. The entire staff were awesome. Assistant Kelly gets a five star stand out for her professionalism and care.

I had a great cleaning experience at the Plymouth office yesterday. I was afraid, but everything was wonderful, no terrible pain after the cleaning. My hygienist took a lot of time to explain to me what she was doing, and was ever so kind and patient with me. I had a new patient appointment today. I have talked with the front desk team several times to get details and confirm my appointment. To be able to make it to this appointment I had to take time off of work and get a sitter for my infant. I get to my appointment and the tech sits me down and lets me know that they can't do anything because they don't have any previous dental records and the front desk already told me that so they don't know why I'm here. WOW. I call my dental office while I'm sitting in the chair and they said they can mail the records to me. They did not have any requests from this office. However, the tech let me know I won't be charged for today so it's not a big deal. I tell her but what about my time and resources? I had to take off of work and get a sitter. I could have easily rescheduled. The tech tells me " they don't care because they have 4 full chairs and aren't concerned about you". The tech further explained, " we have a huddle every morning, and if the front desk cared they would have reached out to you". This office came highly recommended by my family who is current patients. I would never return.
