In 1995 the county Department of Health Administrators of Barry, McDonald, and Newton Counties ( Pat Tichenor, Jerrie Wise, and Mary Ann Bradley) concerned about diminishing state funding for health programs began to collaborate on grant projects that would address their counties health concerns. Out of these discussions, George Thomas from the Bureau of Primary Health Care, a department of what was then called Missouri Department of Health, guided the administrators to form a consortium to write a grant proposal from the Primary Health Care Resources (PRIMO). The grant was designed to provide health care for all residents of these counties regardless of income, ethnicity, and inadequate or lack of health insurance.
That grant and a subsequent grant led to applying to the Bureau of Primary Health Care for Federally Qualified Health Center status. The first executive Director was hired and the office moved to a house in Pineville that served as both the clinic and administrative offices. The clinic functioned as an FQHC Look-Alike until full status was achieved in 1999 when the consortium, Ozark Tri-County Health Care, would have access to a 330 federal grant allowing them to provide sliding fee discounts to qualifying patients . The following year, the clinic moved to a storefront in downtown Anderson with the administrative offices moving just down the same street in another building.
Ozark Tri-County Health Care expanded to provide a medical primary care clinic in Cassville in 2002. It was named in honor of Dr. James D. Warn who served not only in Cassville, but throughout the small communities in southwest Missouri.
In 2002 a dental expansion grant was received and Ozark Dental became a reality in rented facilities in Anderson. Later, a federal grant through the efforts of Missouri Senator Kit Bond and Missouris Seventh District Representative Roy Blunt allowed construction of a new facility combining the medical and dental facilities which was named in honor of the 40+ years of Mary Ann Bradleys service to the residents of McDonald County, The Mary Ann Bradley Medical/Dental clinic, a 6,900 square foot facility, opened in Anderson in a more accessible location in 2004.
Ozark Tri-County Health Care expanded into Joplin in 2004 in rented facilities at 2700 McClelland near both St. Johns and Freeman hospitals and began operating My Clinic, a primary health care facility. The following year, My Dental Clinic opened in other rented space at the same complex.
In 2008, in order to better reflect the geographic and service scope of the health center, the Board of Directors approved changing the name of the organization to ACCESS Family Care. The dream of the board and staff of ACCESS Family Care is to expand dental into Barry and Lawrence Counties.
Ozark Tri-County Health Care consortium purchased a 42,000 square foot building in February 2008 near the intersection of 5th street and Maiden Lane in Joplin to be converted into a Medical and Dental Facility. The renovation began in April 2008 and was completed and moved into in November of 2008. Pediatrics moved to the new location February 2009.
With a HRSA CIP grant and other ARRA funding and donated land, plans were drawn for a new Cassville clinic that would create a new 8,000 square foot health center for three medical providers, and provide new dental services. Expected completion will be during the summer of 2011.
In March 2012 staff moved into a new Cassville clinic. With a HRSA CIP grant and other ARRA funding and donated land, the Cassville clinic became a new 8,000 square foot health center for three medical providers, and two dentists and two dental hygienists as well as other additional new dental services. The completion was celebrated.
In December 2013, ACCESS Family Care received notification that it passed with high marks NCQA Level Three recognition. The National Committy for Quality Assurance is the credentialling body that certifies health centers meet high quality standards of care.
In August of 2015, a grant was awarded to ACCESS Family Care Medical and Dental Clinics to establish a Medical Clinic, in Barton County, Missouri. The Board of Directors purchased property at 210 W. 10th Street, in Lamar, in September of 2015.
On October 1, 2016, ACCESS Family Care Medical & Dental Clinics merged with Area Community Health Emissaries (ACHE) Dental Clinics. This brought three additional Dental Clinics, to the Mission. The ACHE Dental Clinics were located in Nevada, Carthage, and Neosho. They now operate under the ACCESS Family Care name. This was a major stride in our goal to make Dental Care accessible and affordable to the people of Vernon, Barton, Jasper, Newton, McDonald, Barry, and Lawrence Counties, in southwest Missouri.

Some time back, I had a root canal done by these people who were at a different location then. I went to them because they were the only ones that weren't booked solid and I was in serious pain. With my income of the time my co-pay was slightly over $200 which I paid in full in cash. No receipt was given and then I was turned over to collections for nonpayment of the fee I'd already paid.!. To add insult for injury ... literally ... I went to another dentist some time after that and was told, "You've never had a root canal". Actually, I argued with them that I had and they proved to me that I hadn't with the new x-ray "Look, see for yourself, there's the nerve, you've never had a root canal, Sir ... If you'd had a root canal, that nerve would not be there & there it is".!. So, I never had a root canal that I paid for in cash that was never recorded as paid and was sent to collections for nonpayment ... these people are medical criminals at their very best.

Been going here for years. They've made dental work more affordable then any other place I can find. I come from Oklahoma just to get the best dental work. Staff are amazing and the secretary and dentist are the best. The only downside is the very long out schedule. They're always really really far booked out. I've booked out as far as 8months. They did move locations so hoping scheduling gets a little better.

First person I talked to on the phone was extremely nice. Wished she could have been the person to get my kids scheduled, unfortunately it was the second person whose attitude in her job made me not even want to speak with her any longer. I'm so sorry my phone call was only to seek information, I did not need your condescending tone as to me trying to find a new dentist for my child, whose teeth have been hurting, yes, which is why I've been tryinf to find someone. They will never be seen by an access family facility ANYWHERE!! I need a dentist that doesn't hate their job. Thanks.

I drive all the way from Webb City to see this dentist each time. I Absolutely love coming to this dentist, Dr. Greene. She is amazing as well as the rest of her staff whom welcomes me each time I come in and they are all super friendly. I used to not like dentists but they have made me so comfortable, to where I don't even notice anymore. I leave here pain free each time! 10/10 would recommend!

In my 55 years, I've had a lot of preventive dental care and can only say 3 dentists were awesome. This place is golden. I've had a lot of damage due to a high speed moter vehicle seatbelt accident 20 years ago. Unfortunately, i lost some teeth due too crushing. First time I haven't had a dry socketthank you

Dr Green is awesome! She made me feel extremely comfortable, which was great because going to the dentist always gives me serious anxiety. She didn't make me feel judged for the issue that I was having but instead she listened, explained everything to me that she would be doing and she was also extremely friendly and would talk to me about the funniest things to set my mind at ease. This place is also sliding scale so the dental work pricing is based off your income. You will have to wait a long minute to get in but trust me it's worth the wait. If you're in severe pain they will suggest that you call everyday to see if there was a cancelation so you can get in sooner. I will be going back in January but if I'm in any pain, I'll be calling everyday to try to get in sooner lol

I've been there twice for limited checkups. After almost getting scammed at Aspen Dental this place was kind and actually let me know what was going on. Hard to get timely appointments here as an adult, but for a fraction of what i'd have to pay anywhere else it seems worth it. I'm going in for a filling soon and they will be doing 3 crowns on a sliding scale price. I'm hoping that they restore my faith in dentistry.

The nurses are nice but the doctors give you unnecessary fillings. How could I get almost 10 cavities with no pain in the month in between checkups? I've never had a cavity before this. They drilled into my mouth and now I'm miserably sore and achey even months later. I'll never go again.
