Dr. Lyzak was born in Harvey, Illinois on May 12, 1960, the son of Alice Adell and William Arthur Lyzak Sr. After graduating from Homewood-Flossmoor High School, Flossmoor, Illinois in 1978, he entered Southern Illinois University in Carbondale and received the degree of Associate in Arts, Dental Laboratory Technology in May, 1980.
Dr. Lyzak then studied at The University of Texas Systems Cancer Center/M.D. Anderson Hospital and Tumor Institute, Houston where he received a certificate in Maxillofacial Prosthetic Technology in June, 1981. Following this, he returned to Southern Illinois University where he received the degree of Bachelor of Arts, Biological Sciences in May, 1985. Dr. Lyzak entered Dental School at Southern Illinois University in Alton and received the degree Doctor of Dental Medicine in June, 1989. He was in private practice in Springfield, Illinois for two years. In July, 1991, he entered the Postgraduate School of Dentistry at The University of Texas Health Science Center at Houston, Dental Branch, for graduate study in Prosthodontics. Dr. Lyzak received his certificate in Prosthodontics in June, 1993. He then entered The University of Texas M.D. Anderson Cancer Center at Houston, for sub-specialty training in Maxillofacial Prosthetics and Dental Oncology. Dr. Lyzak received his certificate in Maxillofacial Prosthetics and his Master of Science degree in June, 1994. Dr. Lyzak returned to Illinois in June, 1994 to become an Assistant Professor in The University of Illinois at Chicago, College of Dentistry, Department of Restorative Dentistry. He was also named Director of the removable prosthodontic laboratories. In November, 1994 Dr. Lyzak was also appointed an Assistant Professor at The University of Illinois, Chicago Graduate College.
In 1998, Dr. Lyzak left academia and became a member of a multiple doctor practice in Mishawaka, Indiana. In July, 2005 Dr. Lyzak joined Dr. Boldin to become the Co-Owners of Accent Dental in Valparaiso, Indiana. Dr. Lyzak is a member of numerous professional dental organizations including the American Dental Association and the Chicago Dental Society. He is a member of the Omicron Kappa Upsilon Dental Honor Society and has Fellowship status in the American Academy of Maxillofacial Prosthodontics. The research subject of Dr. Lyzaks master thesis was implant dentistry. He received the Golden Apple Award for Teaching Excellence from the University of Illinois, College of Dentistry, Class of 1996. Dr. Lyzak has authored more that 15 publications in peer reviewed professional journals of Dentistry, Medicine and Physiology.

Friendly people from the front of the office to the back! The doctor took time to explain everything to me that I requested.

I can't even comment on the exam itself because I had nothing but billing issues with them. When I originally went I informed them I shouldn't have to pay based on my insurance providing all the details needed, had them pull up my husband's recent visit to confirm the cost and they told me an incorrect amount would be due so I cancelled the appointment and left only to have them call me back in from the parking lot saying they made a mistake so I came back in and had the exam (they actually said that "x person doesn't know how to read the statements" so not sure why that person is working in billing. Fast forward two months later and they charged me for x-rays I never had and a higher bill than my husband had paid claiming costs had increased(yet at checkout I was told I wouldn't owe anything so was shocked when two months later I get a "second notice past due bill") . They also tried to send the same claim twice to my insurance company to see if it would go through so seems like a big scam to just get more money from a patient repeatedly. They don't even know their own costs and with all the clerical errors and repeated mis-information we were both given(even contradicting what they originally told me when stating it again) I cannot recommend this office to anyone.

Dr. Boldin is a top-notch dentist that cares for his patients and is passionate about his profession. While he's almost always busy, you never feel rushed when your appointment comes around since he always takes the time needed to ensure that each patient gets quality care. Although there are many attributes that characterize Dr. Boldin (his intelligence, professionalism, great sense of humor, and down-to-earth nature to just name a few), I think his compassion, attention to detail, and expertise have always made him stand out.

This is one of the best dental offices that I have ever been to. First, they got me in the day I called due to a cracked tooth. I was not even an established patient with them, but my husband was. Everyone there was beyond nice and explained everything they were going to do before they did it. To top it off everyone thanked me for coming in. I have never had that happen. A huge thank you to everyone at Accent Dental and Happy Holidays!

I think that a good dentist is part mechanic, part watch maker, part aesthetician and that those skills accumulate over time. I came to see Dr. Boldin for help with my front tooth-something that is important to me and my smile and felt very comfortable in his hands from the beginning. This is the Dental office that you have been looking for. Extremely professional, yet friendlier than family. I honestly look forward to my visits knowing that I will be happy because of them. Drs. Bolden and Lyzak are the most professional medical people I've ever met . All of the technicians on their staff equally competent and the best in their field. But the thing that stands out in my mind about their practice is the relaxed and incredibly friendly atmosphere . Look around if you want, be this is where you'll want to be. Stop wasting time and make an appointment with people who's goal is to make you happy .

I have been going to Accent Dental since I was a child and I can't imagine anyone other than Dr. Boldin working on my teeth. He is the best. The staff is great and the hygienists Stefanie and Karla are amazing (with adults and kids). Love this place would never think about going anywhere else. 10/10

I have been going to Accent in Valparaiso my experience there has been nothing but positive . I was guided by Karla my hygienist she has been excellent she truly cares what she does . My results have been wonderful as they complimented my diligence in following their directions . I recently have been doing the whitening after having impressions made . Again this was after conversation and counseling from Karla and I have seen very good results ! Thanks !!!

There was a cancellation so I received a call for a much sooner cleaning appointment. Everyone was great! I was taken in promptly at the appointment time. Terra, my hygienist, was very thorough, friendly, and answered my questions. Dr. Lyzak came in right after the cleaning to examine my teeth. The check out was very quick and efficient. While having severe facial pain from what I originally thought was a sinus problem, my brother reached out to his dentist, Dr. Boldin, on my behalf. He told me to call his office first thing on Monday and they would squeeze me in. The entire staff was wonderful and got me in right away for an xray and referral to an endodontist. One day, one abscess and two root canals later I am in a lot less pain and could not be more appreciative of Dr. Boldin and his staff for getting me in so quickly. As long as I have teeth they will have my business!
